Responsibilities
Interprets orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
Accurately compounds and dispenses pharmaceuticals
Complies with facility policy and procedures and regulatory requirements
Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose
Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed
Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selection
Supports participation in daily multidisciplinary and bedside rounds (may need to attend)
Identifies, designs, and analyzes drug utilization reviews to promote rational drug therapy and determines action plan based on results
Provides accurate and timely medication information to healthcare staff and patients
Develops, manages and mentors pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as determined appropriate based on patient population and literature guidelines
Develops, manages and mentors for the antimicrobial management program (AMP)
Qualifications:
B.S. or Pharm D. from an ACPE-accredited School of Pharmacy - Required
Minimum of 2 years as a Pharmacy Clinical/Operations Specialist - Required
Active Pharmacist license in good standing with the appropriate State Board of Pharmacy - Required
Active Pharmacy Consultant license, or meet equivalent Texas requirements for a Pharmacy - Required
Doctor of Pharmacy from an ACPE-accredited college of pharmacy - Preferred
Min 5 years of hospital pharmacy experience required; within the area of focus -NO RETAIL EXPERIENCE
Completion of hospital PGY2/ pharmacy specialty residency or fellowship - Preferred instead of ACU
Specialty BCPS Certification - Preferred
Other certification (e.g. FCCP, FASHP) Preferred Hospital Pharmacy Management experience - Preferred
ACLS certification preferred
